Many patients are surprised when they feel a firm or thick area beneath their incision after surgery. Although it can be concerning at first, this is often a completely normal part of healing.
I commonly see this after procedures such as
tummy tuck surgery,
breast reduction,
body contouring procedures,
and skin surgeries where tissue has been removed or repositioned.
At Oregon Cosmetic and Reconstructive Clinic, I spend a lot of time helping patients understand what they’re feeling, why it happens, and how we can help it improve. Let’s walk through what post-surgical firmness means—and what you can do to support healing.

What Does Scar-Related Firmness Feel Like?
Scar-related swelling often feels like:
- A tough or dense area beneath the skin
- Skin that feels less mobile or “stuck down”
- Firmness that follows the line of the incision
- A tight sensation when stretching or moving
This happens because healing tissue forms temporary connections between the skin and the deeper layers underneath. Your body is laying down collagen to strengthen the area. As healing progresses, those collagen fibers slowly reorganize, allowing the tissue to soften and become more flexible.
Why Does This Happen?
Scar tissue is your body’s natural “repair material.” During the first several months after surgery, collagen is produced quickly to strengthen the incision. Initially, this collagen is thick and disorganized, making the area feel firm. Over time, the fibers remodel and become softer, smoother, and more flexible.
Firmness is especially common after:
- Tummy tuck (abdominoplasty), where the abdominal skin is elevated and redraped.
- Breast reduction surgery, as the breast tissue settles and internal scars mature.
- Body contouring procedures, where larger areas of tissue heal beneath the skin.
- Skin lesion removal and reconstructive skin procedures, especially when deeper tissue is involved.
How Is Scar Firmness Different From Fluid or a Hematoma?
Not every area of swelling is scar tissue. Sometimes swelling develops because fluid collects beneath the skin.
Examples include:
- Seroma – a collection of clear fluid
- Hematoma – a collection of blood
These often feel:
- Soft or squishy
- Like a bubble or fluid pocket
- More sudden in onset
- Larger over a short period of time
Often I can determine the difference during a physical examination. However, healing isn’t always straightforward, and sometimes imaging provides additional reassurance.
When Ultrasound Helps Clarify What’s Going On
When it isn’t immediately clear whether swelling is scar tissue or fluid, ultrasound can be extremely helpful.
Ultrasound is a painless, radiation-free imaging tool that allows me to look beneath the skin in real time. It helps me:
- Identify fluid collections
- Confirm normal scar tissue
- Guide treatment decisions
- Avoid unnecessary procedures when everything is healing normally
Because ultrasound is quick and performed right in the clinic, it often provides immediate reassurance and answers.
Treatment: Scar Firmness vs Fluid Collections
Treatment depends entirely on what is causing the swelling.
Compression
Compression garments help reduce swelling while supporting healing tissues after procedures like tummy tuck, breast surgery, and body contouring.
Scar Massage
Gentle scar massage improves tissue mobility and helps soften thicker scar bands as healing progresses.
Lymphatic Massage
Manual lymphatic massage encourages excess fluid to move through the lymphatic system, helping decrease swelling and improve comfort.
Ultrasound Evaluation
If swelling seems unusual, ultrasound helps distinguish scar tissue from fluid collections so treatment can be tailored appropriately.
If the Swelling Is Actually Fluid
Fluid collections may require:
- Observation
- Compression
- Drainage when appropriate
- Repeat follow-up examinations
That’s why identifying the cause is so important—the treatment pathway can be very different.
When Should You Contact Your Surgeon?
Although firmness is usually part of normal healing, you should contact your surgeon if you notice:
- Rapidly increasing swelling
- Increasing redness
- Drainage from the incision
- Increasing pain after initially improving
- Fever or chills
- A new enlarging soft fluid pocket
These symptoms don’t always indicate a serious problem, but they deserve an evaluation.
Healing Takes Time—and That’s Normal
Scar tissue changes slowly. Early firmness does not mean something is wrong. Instead, it usually reflects your body’s normal healing process.

- Carefully evaluate any unexpected changes
- Support healing using the least invasive treatment possible
- Help you feel confident throughout recovery
