Breast Feminization Surgery, also known as breast augmentation for transgender women, is a gender-affirming procedure that enhances chest contour using breast implants or fat transfer. Who is a Good Candidate for Breast Feminization Surgery?

You may be an ideal candidate if you:

  • Are a transgender woman or non-binary individual seeking a more feminine chest contour.
  • Have been on hormone replacement therapy (HRT) for at least 12 months (recommended but not required).
  • Are in good overall health and have realistic expectations.
  • Want to achieve a natural-looking, proportionate breast shape.

Silicone vs. Saline Implants: Pros and Cons

Deciding between silicone and saline implants is an important step. Here are the basics:

Silicone Implants:

  • Pros: More natural look and feel, especially for thin patients. Less chance of visible rippling.
  • Cons: Harder to detect rupture; regular MRI checks recommended and changing out implants at 10 years recommended.

Saline Implants:

  • Pros: Easy to detect rupture since the implant visibly deflates. Smaller incision possible.
  • Cons: Might feel less natural, with a higher chance of visible rippling.

For treatment of gender dysphoria, we generally recommend silicone implants due to the more natural feel and look.  

Implant Placement Options

Implants can be placed in different positions, each with its advantages and disadvantages:

  • Below the fascia of the pectoralis muscle (subfascial):
    • Pros: Balances natural look and muscle protection, reduces visibility.
    • Cons: Still some risk of implant visibility and slightly longer recovery.
  • Below the pectoralis muscle (submuscular):
    • Pros: Provides a very natural look with more support for the implant, lowest chance of rippling, and clearer mammograms.
    • Cons: Longer recovery time, more initial discomfort, and possible muscle animation during movement.

Risks and Considerations

  • Implants aren’t permanent; future revisions might be needed.
  • Possible complications include implant rupture, capsular contracture, or shifting.
  • Rare cancer risk (BIA-ALCL) mainly with textured implants. We use only smooth implants to minimize this risk.

Surgery and Recovery

Breast surgery typically takes about 2 hours. The surgery is outpatient, meaning you’ll go home the same day. We typically use an incision in the inframammary fold (beneath the breast), which hides well and provides straightforward access for any future surgeries. Dr. Jenq does not use drains during surgery.

Most patients recover within about 2 weeks. Swelling and discomfort are common at first but improve quickly. Following Dr. Jenq’s recovery instructions helps you heal comfortably and safely.
