Gender Affirming Top Surgery

Affirming Care

At Oregon Cosmetic and Reconstructive Clinic, we provide caring and inclusive support for transgender and nonbinary people seeking top surgery in the Portland Oregon Metro Area. Our team is led by Dr. Tina Jenq, board-certified plastic surgeon with over 15 years of experience.  In this clinic, a calm space places your comfort, goals, and well-being first. Dr. Jenq specializes in gender-affirming top surgery and chest contouring. In addition, our clinic provides a peaceful setting and gives you personalized care every step of the way.

Whether you’re just beginning your journey or already know what you want, we’re here to guide you. This decision is personal. So Dr. Jenq works closely with you to create a surgical plan that matches your body and your identity.

Male-to-Female (MTF) Top Surgery

For those looking to masculine their chest, we offer

  • Double incision mastectomy (with or without nipple grafts) – ideal for most patients with excess skin and tissue
  • Keyhole and periareolar top surgery – small chest sizes with minimal excess skin or tissue
  • Subcutaneous mastectomy with lower chest scar – for smaller chest sizes with some excess skin to be removed just at the inframammary fold

Female-to-Male (FTM) Top Surgery

If you’re looking to feminize your chest, breast augmentation may be the right choice.

  • Options to consider include saline or silicone implants
  • Another option to discuss is the layer of placement for the implants – over or under the muscle of the pectoralis

Gender Affirming Nonbinary Chest Surgery

  • Double incision mastectomy –  for patients with excess skin and tissue that prefer a flat chest
  • Breast Reduction – some patients desire the aesthetics of a smaller breast that is more congruent with their identity

Nipple Reconstruction

  • Nipple graft – A smaller, less projected nipple and areola, from tissue reconstructed of areolar skin
  • Delayed nipple reconstruction – For patients that chose not to have a nipple graft with their prior top surgery, nipple reconstruction is still possible by using small flaps to create a projected nipple.

What Sets Our Practice Apart

Choosing a surgeon is an important decision—and we believe you deserve to know who will be performing your procedure. At our clinic, Dr. Jenq performs every part of the surgery herself, from start to finish. Unlike some practices where surgical tasks may be delegated to trainees, residents, or surgical technicians, your care will never be passed off to someone else. This direct attention not only supports safety and consistency but ensures that your experience is truly individualized.

Another key benefit of our approach is that we do not use surgical drains. Many patients are relieved to learn this, as drains can be uncomfortable and can prolong recovery. With advanced techniques, Dr. Jenq is able to minimize fluid buildup and support a smoother healing process—without the need for external drains.

Additionally, all of our top surgeries are performed as outpatient procedures, meaning you’ll go home the same day. Most surgeries are completed in under three hours, and patients can typically return to light daily activities within a week. We understand the importance of clear recovery expectations and are here to support you through every phase.

Payment Options That Fit Your Needs

We recognize that affordability and access are essential. That’s why we offer payment options, including both insurance and self-pay. Our experienced staff helps you navigate the insurance approval process and provides clear, upfront information about costs if you choose to pay out of pocket. We’re value transparency and work with you to make sure there are no surprises along the way.

Frequently Asked Questions

What is your wait time from initial consultation to surgery?

We offer timely access to care. Most people can have surgery 1–2 months after their first visit with us.

What are drains, and what are the pros and cons of using them?

Drains are small tubes that remove fluid after surgery. They may help healing but can feel uncomfortable or hard to manage. Instead, our method of quilting sutures avoids drains. This keeps skin flat and prevents fluid buildup also. While the surgery takes about 20 minutes longer, it worth the improved comfort patients experience. In this practice, we have not seen an increase in fluid collection complications after stopping using drains.

Do I need a letter from a therapist?

You don't need a letter to attend your first consultation. This allows you to get more information, understand the process, and go over your goals. However if you decide to have surgery, a letter from a licensed mental health provider is required. This helps make sure you feel safe, ready, and supported in your journey. Also, if you need of help finding a therapist, we can share some trusted resources.
