Save Time Before Your Visit

If you are new to our clinic, you can reduce waiting time by downloading the medical form on the right and completing it before your arrival.

Please remember to bring your driver’s license and insurance card on the day of your visit. We do have a $25 cancellation fee policy if you cancel less than 24 hours before your appointment.
